The Shelter Outreach Worker provides "Common Ground" Parent/Teen Mediation, one to one support and success coaching for youth and families in order to prevent youth homelessness. When a community youth does access the shelter, the Shelter Outreach Worker collaborates with all stakeholders to help transition the youth back home or to a stable living arrangement and provides follow-up support to ensure the placement is maintained
